1) Complete and submit the entire FSU Application for Admission.
You may complete and submit the FSU Online Application. |

2) Submit the required $30.00 application fee.
This fee is required for the processing of the application. You can pay by phone, online, or send a check or money order. If paying with a check or money order,
please submit the Application Fee Transmittal Form (PDF) with your
payment. You can print the PDF version of the form now or you can wait and print a copy of the form after
completing the online application. Please include a personal check or money order for
$30 made payable to Frostburg State University and place the student's name and social
security number on the check or money order. Send payment and the completed Application Fee Transmittal Form (PDF) to the Graduate Services Office. If you have previously paid an
application fee to Frostburg State University, you do not need to submit this fee a
second time. DO NOT SEND CASH. |

3) Submit the Request for Academic Record.
This form should be
submitted to all schools attended for completion. |

4) Submit official transcripts. Only official transcripts are acceptable. Official transcripts are those that are released with the school's seal and the signature of the appropriate official in a sealed envelope. Transcripts should be sent directly from the school to the Frostburg State University Graduate Services Office. |
